RE Classes SMC Offers:
We offer four (4) options for your children, be sure to review the options and select the correct class(es) for your children.
If you have questions, please contact at the Formation office.
#1Needed documents
First registrant will be the parent/legal guardian
Each registrant will need to provide:
- Birth Certificate
- Sacramental records (i.e., baptismal cert., etc)
NOTE: This form will register each student and their parent/legal guardian.
#2Student and parent MUST register!
It is mandatory that both the student(s) and the parents use this form to register.
A Parsishoner ID is NOT mandatory.
Reach out to the Formation staff if you have questions.
#3Save your form
To insure your registration has been saved, look for the blue button. If it is not blue, more information is needed.
#4Register your student next
If you are registering more than one student, only one form is needed.
#5Student Registration
You are now ready to add your student to the registration form.
#6Select your class
Choose the class from the four options offered. Pick the class that best suits each student.
